PAPER MATERIALS (REGULAR PRINT, LARGE PRINT, BRAILLE, IDAA RESPONSE OPTION CARDS) ORDER FORM

The district must fill out this form for each student requiring accommodated test materials for the ISAT (ELA/Literacy and Math) and/or Idaho Alternate Assessments (ELA and Math). These materials must be ordered by a District Test Administrator (DA) or a District Test Coordinator (DC). Because of processing time, paper test material must be ordered by Tuesday, February 16 in order to guarantee receipt prior to the opening of the paper test window on Monday, April 5. Materials will continue to be available to order through Wednesday, May 12. Any orders past February 16 will not be guaranteed for arrival prior to the testing window.
